The HBC programme was initiated in 2002 at the time when the HIV/AIDS epidemic became prevalent. Many elderly, either sick and/or stranded were found in their homes during our outreaches in the community. In the beginning it was generalized HBC, but is now focused on the elderly (60 years and above) since 2014. The aim is to nurse patients in their own homes, providing the necessary medical assistance required, and in the case of TB, D.O.T. support to all the sick at home. In situations where poverty is the order of the day, the elderly are extremely vulnerable.
These programmes have turned people’s lives around in a holistic and tangible manner. Patients are attended to in their homes by trained Home Based Care Workers and the Social Worker.
The project has recently expanded to include Adherence Clubs. Patients with chronic illness receive their medication, their health is monitored and they are motivated to live a healthy lifestyle. Trained professionals are available for counselling.
